How to join the SWH network
To become a credentialed Senior Whole Health provider, please submit a completed
Provider Data Form

The credentialing process

Our credentialing team will review and process your request. We may contact you to obtain additional information required to complete your application.  If you are a solo practitioner or part of a non-participating group, you will be required to sign a Senior Whole Health Provider Agreement. A representative from our Provider Relations Department will contact you regarding your next steps.

Please note: Senior Whole Health cannot guarantee that requesting providers will become contracted providers. Providers are subject to reviews based on network adequacy and Senior Whole Health qualification standards.

Credentialing is finalized when approved by the Medical Advisory Committee and by the return of a countersigned Agreement (if applicable) along with an accompanying letter identifying you as a credentialed provider.

In order to prescribe Part D drugs to Medicare patients, you must be enrolled as a Medicare and Medicaid provider.
